As healthcare professionals, we realized during the COVID-19 pandemic how unprepared we all were. We saw patients who were admitted to our facilities who are too sick to provide a relevant medical history. Since visitation was restricted, family members were unable to be with their loved ones during their sickest time and could not be there to share medical history with the clinicians who were tasked with caring for them.
We wanted to create a tool that would ease the burden of gathering medical information which at times can be taxing. It is our prayer that this invaluable tool will aid in the job our healthcare providers do every day and allow them to obtain medical history, emergency contact and allergy data in a timely manner.
We are committed to protecting your privacy. Information collected about our clients is not shared with or sold to any other organization. We only accept payments. We do not have access to any credit/debit cards that you use.

Returns can be made for 100% refund within 30 days of purchase minus postage and handling. Once the card has been returned, refunds should be available within 7-10 business days.
